Child Health Associates participates with many insurance companies. Your insurance contract is between you and the insurance company. Each employer has a different set of benefits, deductibles, co-pays and maximums. Do not assume that your insurance company will pay for any service rendered in our offices. It is your responsibility to know your insurance plan so that you are aware of your financial responsibilities. Our billing department will make every effort to process an insurance claim, but we are not responsible for disputes between you and your insurance company. Payment for services is your responsibility.

Please help us by doing the following:
- Always bring your insurance card with you to the office
- After the birth of a child, call your employer and inform them so that your child will be placed on your insurance policy. This does not occur automatically!
- Advise us of any special restrictions with regards to laboratory benefits. Some insurance companies will not allow us to do labs in the office.
- Always retain your receipts and explanation of benefits that your insurance company provides.
- If a claim is rejected, call your insurance company first. Most questions regarding non-payment of a claim can be answered by them. It is not our responsibility to mediate a dispute between you and your insurance carrier
- Bring your copay to the office. We accept cash, checks and Visa/MasterCard/Discover/American Express. Copays can not be billed and failure to pay at time of service will result in a $20 service charge.
