Founded in 1975, Child Health Associates, P.C. has a long history of providing evidenced based, high quality care to the children of the Detroit metropolitan area. Our practice is comprised of board certified providers with extensive experience. We focus on creating strong relationships with each family in order to provide the best advice, recommendations and treatments. We make it our mission to improve the health of each of our patients and deliver care with the highest degree of professionalism, compassion, and personal service we would expect for our own children and families. We are dedicated to the education of parents, with the goal of empowering them to be informed partners in the care of their children. Patient Centered Medical Home
Child Health Associates is a designated “Patient Centered Medical Home.” We have an ongoing commitment to partner with each of our families in order to provide exceptional care. This partnership contains responsibilities for us, your health care providers, and you, our families.
Responsibility of Your Primary Care Provider
- Work with each patient and family to improve health
- Review and manage medications at each visit
- Develop a plan to manage any health problem, including chronic health problems
- Use computerized technology to optimize care
- Inform patients/families of test results in a timely manner
- Provide educational material and information about community resources
- Provide 24 hour phone access to a medically trained professional
- Work with after hours urgent care and emergency centers to be informed of patient visits
- Obtain consult notes from specialists in a timely manner to ensure consistency of care
- Offer same day appointments
Responsibility of Patient Families
- Make sure health care provider knows my child’s entire medical history including medications and allergies
- Actively participate with health care provider when planning care
- Keep appointments as scheduled
- Follow health care provider recommendations
- Utilize patient portal to review messages and communicate with my health care provider
- Complete forms and information sent by my health care provider in a timely manner
- Ask health care provider for further explanation
- Ask primary health care provider for guidance before making a specialist appointment
- Provide the office feedback on how care can be improved
